You are hereby notified that a Petition under oath has been filed in the above-styled Court for the termination of your parental rights that you may have as a named prospective biological father to J.B., a male child born on September 4, 2009, in Jefferson County, Kentucky and M.B.T., Jr., a male child born on May 26, 2008, in Jefferson County, Kentucky, from the mother, Sondra Hernandez, and commitment of these children to the State of Florida Department of Children and Families for subsequent adoption. You are hereby noticed and commanded to be and appear before the Honorable Christopher LaBruzzo, Judge of the Circuit Court, at the Pinellas County Justice Center, 14250 49th Street North, Annex, Courtroom A, Clearwater, Florida, 33762, on Wednesday, September 27, 2023, at 10:00 a.m.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF THESE CHILDREN. IF YOU FAIL TO PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO THESE CHILDREN NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
