YOU ARE NOTIFIED that an action for Declaratory Judgment has been filed against you, individually and as parent and guardian of Lekian Perez, a minor, to have the Equity Insurance Company insurance policy #P0500104593-04 that was issued to MARIA LUKEFAHR with an effective date of May 14, 2022 declared null and void for misrepresentations, omissions, concealment and incorrect statements that were material to the acceptance of the risk and the hazards assumed. You are further notified that you are required to serve a copy of your written defenses, if any, to Robert E. O'Connell, Esq., Plaintiff's attorney, whose address is P. O. Box #5084, Deerfield Beach, FL 33442, telephone number (561) 999-3250, on or before 01/08/ 2024 and file the original with the Clerk of the Court.
If you fail to do so, a default will be entered against you individually and as the parent and guardian of Lukian Perez, a minor, for the relief demanded in the Complaint.
This notice shall be published once a week for four consecutive weeks in the Business Observer or other newspaper which has circulation in Pasco County.
