DENNIS MADDOX A/K/A WALLACE MADDOX A/K/A WALLACE D. MADDOX A/K/A WALLACE DENNIS MADDOX A/K/A W. D. MADDOX; STATE OF FLORIDA, DEPARTMENT OF REVENUE; HERTZ ENTERPRISE; CLERK OF THE COURT OF POLK COUNTY, FLORIDA; KENDRA MADDOX A/K/A KENDRA L.
