You are hereby notified that a Petition under oath has been filed in the above-styled Court for the termination of your parental rights and commitment of the child for subsequent adoption of K.M. a male child, born on January 11, 2013 in Greene County, Tennessee to the mother, Susan Malone. You are hereby commanded to appear before a Judge of the Circuit Court, the Honorable Kimberly Campbell, Juvenile Division, in and for the County of Pinellas, and State of Florida, at the Criminal Justice Center, Annex Courtroom C, 14250 49th Street North, Clearwater, Florida 33762 on Tuesday, September 10, 2024 at 9:00 a.m., for an Advisory/ Adjudicatory/Disposition hearing on the Petition for Termination of Parental Rights.
FAILURE TO PERSONALLY APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE TERMINATION OF YOUR PARENTAL RIGHTS OF THIS CHILD. IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL LEGAL RIGHTS AS A PARENT TO THE CHILD NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
