Whereas a Petition under oath has been filed in this Court alleging the above-named child to be dependent and requesting termination of parental rights under the laws of the State of Florida, a copy of which is attached hereto; you are hereby commanded to appear before the Honorable James Pierce, Judge of the Circuit Court, Pinellas County, Florida, at the Pinellas County Criminal Justice Center, Annex B, 14250 49th Street North, Clearwater, Florida, 33762 on Thursday, September 19, 2024 at 9:30 a.m., to answer to the Petition alleging that your parental rights to your child should be terminated and to be dealt with according to the law.
FAILURE TO PERSONALLY APPEAR AT THIS ADVISORY HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF YOUR CHILD. IF YOU FAIL TO PERSONALLY APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO YOUR CHILD NAMED IN THE PETITION. AN ATTORNEY CANNOT APPEAR FOR YOU.
