YOU ARE NOTIFIED that an action for Supplemental Petition to Modify Child Support and Petition to Establish Parenting Plan (with Time-sharing Schedule) Unconnected with Dissolution of Marriage in a Case which the Florida Department of Revenue (DOR) is a Party has been filed against you and that you are required to serve a copy of your written defenses, if any, to it on EVA DANIELLE MOORE, whose address is 4604 49th STREET NORTH #75, PINELLAS PARK, FL, 33709 on or before ON OR BEFORE 20 DAYS AFTER 1st PUBLICATION and file the original with the clerk of this Court at 545 1st Ave N, St. Petersburg, Florida 33703., before service on the Petitioner or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.
Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.
You must keep the Clerk of the Circuit Court's office notified of your current address.
