Deceased.
BEFORE ME, the undersigned authority, personally appeared Jaime Duncanson, who, after being by me duly sworn, deposes and says:
1. Affiant, Jaime Duncanson, is a cousin of the Decedent and is the Personal Representative of the Estate of Michael P. O'Malley.
2. The Decedent, Michael P. O'Malley had one child, to wit:
a. Michael T. O'Malley.
3. Affiant does not know the whereabouts of her first cousin once removed, Michael T. O'Malley.
4. Affiant has not communicated with or known the whereabouts of her first cousin once removed, Michael T. O'Malley for over 20 years.
5. Affiant has attempted to locate her first cousin once removed, Michael T. O'Malley through the world wide web and social media.
6. Furthermore, Affiant has contacted numerous friends and family members to try to track down and/or locate Michael T. O'Malley.
7. None of the family members contacted were able to provide any information or provide the location of Michael T. O'Malley.
8. Affiant believes the last known whereabouts of Michael T. O'Malley to be in Galien, Michigan and it appears he does not want to speak to anyone associated with his father's family or this matter.
9. Affiant's attorney has also conducted a Thompson Reuters Westlaw Public Records Search “People Search” for Michael T. O'Malley and have sent letters via regular and certified mail to the address located in Galien, Michigan, however, they all were not responded to or returned unclaimed.
