YOU ARE HEREBY notified that a Petition for Termination of Parental Rights has been filed by the State of Florida Department of Children and Families, seeking to terminate your parental rights to the child, D.A., born on February 26, 2024. Whereas the State of Florida has filed in this Court a Petition for Termination of Parental Rights.
You are commanded to appear for an advisory hearing before this Court at the Criminal Justice Center, Annex Courtroom B, 14250 49th Street North, Clearwater, Florida, 33762, on Thursday, December 4, 2025 at 9:30 a.m. to answer to a Petition alleging that your parental rights should be terminated and to be dealt with according to the law.
FAILURE TO PERSONALLY APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE TERMINATION OF PARENTAL RIGHTS OF THESE CHILDREN. IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED YOU MAY LOSE ALL YOUR LEGAL RIGHTS AS A PARENT TO THESE CHILDREN NAMED IN THE PETITION ATTACHED TO THIS NOTICE. AN ATTORNEY CANNOT APPEAR FOR YOU.
YOU HAVE THE RIGHT TO BE REPRESENTED BY A LAWYER. IF YOU CANNOT AFFORD ONE, THE COURT WILL APPOINT ONE FOR YOU.
