Matthews, Esquire, attorney for Petitioner, whose address is 1091 East Brandon Blvd., Brandon, Florida 33511, and file the original with the clerk of the above styled court at 800 East Twiggs Street, Tampa, Florida 33602, before service on attorney for Petitioner or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.
