Pursuant to Section 456.057, Florida Statutes, and Rule 64B2-17.006, Florida Administrative Code, notice is hereby given that HJCSquared, LLC d/b/a Harbor Chiropractic of Palmetto has relocated its chiropractic practice. The practice is now operating as: RHC Health Services, LLC d/b/a Harbor Chiropractic at 1813 Manatee Ave W, Bradenton, FL 34205. RHC Health Services, LLC d/b/a Harbor Chiropractic is the new custodian of all patient medical and chiropractic records previously maintained by HJCSquared, LLC d/b/a Harbor Chiropractic of Palmetto. Patients may obtain copies of their medical records or request transfer of records by submitting a written request to: RHC Health Services, LLC d/b/a Harbor Chiropractic at 1813 Manatee Ave W, Bradenton, FL 34205. Copies will be provided in accordance with Florida law.
