Petitioner, MARK O’CONNOR, alleges:
1. Petitioner has an interest in the above estate as Brother. Petitioner’s address is 2 Emerson Point New Rochelle, NY 10808, and the name and office address of petitioner’s attorney are set forth at the end of this petition.
2. Decedent, DOUGLAS T. O’CONNOR, whose last known address was 2842 Tanglewydle Dr, Land O’ Lakes Fl 34638 and the last four digits of whose social security number are 9445, died on April 21, 2026, at Avent Health Wesley Chapel. On the date of death decedent was domiciled in Pasco County, Florida.
3. So far as is known, the names of the beneficiaries of this estate and of the decedent’s surviving spouse, if any, their addresses and relationships to decedent, and the years of birth of any who are minors, are:

4. Venue of this proceeding is in this county because it was the county of the decedent’s residence at the time of the decedent’s death.
5. Petitioner, MARK G O’CONNOR, whose address is 2 Emerson Point New Rochelle, Ny 10808 is qualified under the laws of the State of Florida to serve as personal representative of the decedent’s estate and is entitled to preference in appointment as personal representative because all of the heirs of the estate have consented to Petitioner as personal representative. The consents have been filed with this Petition. 6. The nature and approximate value of the assets in this estate are:

9. The decedent’s last will dated November 03, 2016, and codicils are in the possession of the court or accompanies this petition. An authenticated copy of a will and/or codicil deposited with or probated in another jurisdiction accompanies this petition.
An authenticated copy of a notarial will or codicil, the original of which is in the possession of a foreign notary, accompanies this petition.
10. Petitioner is unaware of any unrevoked will or codicil of decedent other than as set forth in paragraph 9.
11. The nature and approximate value of the assets in the estate are:
