YOU ARE NOTIFIED that a Supplemental Petition to Modify Custody has been filed in Collier County, FL and that you are required to serve a copy of your written defenses, if any, to it on Angel Sindo, at 249 Deerwood Circle, Naples FL 34113 on or before {date}, and file the original with the clerk of this Court at Collier County Clerk of the Circuit Court, 3315 Tamiami Trail East, Ste. 102 Naples, FL 34112-5324 before service on Petitioner or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.
The action is asking the court to decide on:
1. Children issue. Modifying Custody of Azariah Jesus Lattendresse DOB: 1/8/2016
2. Real or personal property. The parties have none.
3. Spousal Support: None is requested
